#021. Oral GLP-1 (small molecule) #1

최근 small molecule GLP-1 관심을 가졌었음.

GLP-1 개발 흐름을 보면

QD dosing → QW dosing → Longer Dosing or small molecule?

 

GLP-1 외 다른 기전으로 확대도 있지만

Peptide 및 injector 생산 문제인지 Small molecule Oral GLP-1를 보려는 느낌이 큼.

(아무래도 Obesity 치료제로 하려면 값 싸게 small molecule 만드는게 접근성도 그렇고 편의성도 그렇고)

 

초기에 small molecule 기반 GLP-1 agonist들이 AE도 많고

Lotiglipron은 liver enzyme elevation으로 중단되고

Danuglipron은 AE가 많고 discontinuation으로 23년 연말에 VKTX를 열심히 공부했었는데

아무래도 비만의 접근법을 하려면 small molecule 관심도 있어야 될 것 같음.

 

 

가장 앞서 나가있는 물질이  Lilly의 Orforglipron

비만 대상 임상시험 3상 등록 속도가 어마어마함. (clinicaltrials.gov로 가져오는 날짜라 틀릴 가능성도 있음)

25년 하반기면 결과를 자랑스럽게 낼듯.

best-in-class가 아니더라도 다른 물질도 시장에서 주목할것 같으니, 대안을 찾아보는 걸로

 

개발중인 Small molecule GLP-1;

Lotiglipron은 Liver Enzyme Elevation으로 개발중단되고

Danuglipron도 BID로 하다가 discontinuation rate이 높아서 기존 계획을 바꿔서 QD dosing 탐색중

(어째 모양새가 Danuglipron도 접게 될듯)

그렇게 되면;

• CT-388 (Roche에서 코드명 모르겠다)
• ECC5004 (AZ에서 코드명 모르겠다)
• GSBR-1290
• TERN-601

 

어차피 Roche나 AstraZeneca가 GLP-1 개발 성공한다고 나오는 beta가 얼마 안되니

Pfizer의 실패 사례를 참고하고 GLP-1 공부를 해서 GSBR-1290 / TERN-601 中에 관심 갖을 필요가 있을지 공부

Danuglipron BID 중단 사유:

High rates were observed (up to 73% nausea; up to 47% vomiting; up to 25% diarrhea).

High discontinuation rates, greater than 50%, were seen across all doses compared to approximately 40% with placebo

(Placebo도 40%나 되는 이유는 뭐지?)

Danuglipron QD dosing의 PK데이터는 2024년 상반기 예상이라고 했었음.
