장기 FU이 나왔기 때문에 1년 이상 경과를 죄다 볼 수 있음.

15명 환자 대상으로 진행

• SLE: 8명
• Idiopathic Inflammatory Myositis: 3명
• Systemic Sclerosis: 4명

 

이번에는 Median FU: 15개월

• 모든 SLE 환자들 DORIS remission (DORIS Remission이 뭐였지?)
• 모든 idiopathic inflammatory myositis 환자들 ACR-EULAR major clinical response
• 모든 Systemic Sclerosis 환자들 EUSTAR activity index 감소

주목할 부분은 여전히 모든 CAR-T 투여 환자들 medication free!

 

다시 한번 상기시키는 용도:

MB-CART19.1 (제조방식 및 자세한 내용은 supplementary appendix 참고)

모든 환자대상으로 1x10^9 CAR-T 이상 생성 가능했음.

Transduction efficiency: 18%-44%

 

과거 B-cell depleting antibody 치료를 받은 환자에 대해서:

• Baseline B-cell 숫자가 낮았음.
• B-cell aplasia 기간도 길었음
• CAR-T 세포숫자 peak도 낮았음. [과거 rituximab 이력O 91 cells/um vs. 이력X 461 cells/um]
• CAR-T 혈액 내 유지기간도 짧았음. [과거 rituximab 이력O 40 days vs. 이력X 58 days]
  (과거 Rituximab 치료이력에 따라 CAR-T에게 영향을 줬는데 치료 효과에서 차이를 보이나?)
  Discussion에서 치료효과에는 차이가 없다고 되어있음.

SLE 환자 (8명):

• 29개월까지 FU도 존재했는데, 모든 SLE 환자 SLEDAI-2K 점수 0점 / anti-dsDNA antibody negative 유지 / C3 level normal 유지 / Proteinuria도 사라짐 (쩌는데?)
• 1년 이상 Follow-up 한 환자 7명에서 SLE관련 autoantibody level NEGATIVE 유지 (개쩌는데?)

 

idiopathic inflammatory myositis (3명):

• ACR-EULAR major clinical response (이거 기준이 뭐지?)
• Creatine Kinase level normalisation (투여 3개월 이후부터 FU기간까지 normal level 유지)
• MMT-8 (muscular function) normalised

 

Systemic Sclerosis (4명):

• EUSTAR activity index & skin activity 모두 감소 (modified Rodnan skin score 기준) (이거 기준이 뭐지?)
• 6개월 이상 Follow-up한 3명의 환자에게서:
  ▶ EUSTAR activity index: -4.2 points
  ▶ Modified Rodnan Skin score: -9 points

 

Anti-nuclear & Anti-Ro60 autoantibody 감소 (3개 질병 모두다 해당되는듯?)

 

15명 환자 모두 glucocorticoid & 면역억제제 중단 (쩌는데?)

Protective IgG는 큰 영향 없는듯. CAR-T 투여 이후 백신접종에 대해서도 IgG level 증가

 

Safety에 대한 언급은 short-term & long-term 모두 뭐 딱히 문제 없음.

B-cell Lineage에서 발현 범위:

Discussion에서 언급하는 부분:

• B-cell reconstitution되고 2년 지나도 relapse 없었음.
• Autoantibody 없거나 감소 수치 유지.
• Appearance of naiive non-class switched B-cell (이 변화가 갖는 의미?)
• Disappearance of plasmablasts (이 변화가 갖는 의미?)
• Downregulation of disease-specific Heavy and Light chains (무슨뜻?)
• Lymphodepletion으로 인한 효과도 배재할 수 없지만, B-cell 변화를 바탕으로 했을 때 lymphodepletion만의 효과로 보기는 어려움.
• Infection 발생 적었고 total Ig는 moderately decreased.
• Long-lived plasma cell은 anti-CD19 CAR-T에 대한 영향이 적어서 new vaccination에 대해 면역보호 반응이 어떨지 봐야한다고 적음.
  (결국 autoantibody vs. protective antibody 생성세포를 구분을 잘 모르겠네)

1년 이상 장기 Follow-up 진행한 SLE환자에서의 antibody 변화:

A: autoantibody level in SLE 환자 7명 (1년 이상 FU 경과한 환자들)

B: vaccination antibody level in  SLE 환자 7명 (1년 이상 FU 경과한 환자들)
